YOU MUST HAVE ALL 3 OR WE WILL NOT BE ABLE TO PIERCE YOUR MINOR
(NO EXCEPTIONS)
- Valid Legal ID for Parent/Legal Guardian
- Photo ID for Minor
- Driver’s License/Learners Permit
- State ID/Tribal ID
- Passport
- SmileSafe Card
- School ID
- Alternate Option: If none of these are available, parents may login to online school portal and take a picture where the minor’s name and photo are listed.
- Birth Certificate or Guardianship Form
Legal names MUST match names listed on IDs. If name has changed, a legal name change form must also be provided.
